How to use it
- Fill in the customer and the vehicle before the keys leave the counter. Read the VIN off the door jamb or the base of the windshield, and write the mileage on the odometer today.
- Write the complaint in the customer's words, not the diagnosis.
- Put what the technician found under Diagnostics. The work goes on labor and parts lines, one line each.
- Get the customer's yes before the work starts, and mark which lines they approved.
- Rules on what a repair order must show differ by state. Check your state's auto repair rules; this sheet does not say what yours requires.

Questions shops ask
What goes on a repair order?
The customer, the vehicle with its VIN and mileage, the complaint, what was found, the labor and parts lines, and the total.
Is a repair order the same as a work order?
They name the same record. Auto repair shops mostly say repair order; diesel and fleet shops often say work order.
Should the complaint be in the customer's words?
Yes. Write what they say is wrong; what the technician found goes in its own place.
Can I use this template for free?
Yes. Print as many as you like; nothing is saved or sent.

The same record, on every job
In Scarab OS each visit is one repair order, from the write-up to a sealed record that nobody can change without it showing.
